The trust that runs mental health services in Norfolk and Suffolk has been put back into special measures following an inspection by the Care Quality Commission.
It comes after serious concerns were raised over patient safety, staff levels and leadership and Norfolk and Suffolk Foundation Trust (NSFT) earlier this year.
The trust whose HQ is in Hellesdon, was originally placed into special measures in 2014, but came out of it a year ago.
In the latest inspection it was found it had not maintained improvements and has now rated it as an inadequate.
The Trust said they accept the new findings and will try to put things right. Their full statement is below.
