An inquest heard how Victoria Harrison, who was 17, died in Kettering General Hospital following routine surgery that went wrong.
Recording a narrative verdict the coroner said there had been a catalogue of fatal errors.
The family of Victoria say it's been a harrowing couple of days - hearing evidence into the inquest of her death.

A teenager from Northampton bled to death in Kettering General Hospital following routine surgery that went wrong.
An inquest in Corby has heard that Victoria Harrison, who was 17, died after a surgeon tore an artery during an operation.
The inquest heard it took 15 hours for her to die. The coroner today recorded a narrative verdict. Victoria's death has left her family devastated.
