Norwich City striker Cameron Jerome join Derby County in a deal reported to be worth around £2m.
He scored 42 goals in 138 appearances and hit the back of the net in the Canaries' 2-0 play-off final win at Wembley.
City Head Coach Daniel Farke told the club's website: "It was a tricky decision. He has done so much for this club and always had a good attitude, so we respect what he wanted to do. With Cameron going, there is space in the squad. We are looking to strengthen in the striking areas."

Norwich City’s club captain Russell Martin has joined Rangers on loan until the end of the season.
Defender Martin, 32, said he was keen to get back to playing regular football as he tries to earn a recall to the Scotland squad.
Martin is 22nd on Norwich’s all-time appearances list, which includes 125 appearances in the Premier League – a club record.
He has been involved in three promotions with City and has 308 appearances for the Canaries, scoring 17 goals.
Northampton Town have signed defender Jordan Turnbull from Coventry City on a two and a half year deal.
The 23-year-old has played for England at under 19 and under 20 levels, and spent two season-long loan spells at Swindon Town.
Turnbull moved to Coventry City in August 2016, making 46 appearances for the Sky Blues and scoring twice.

Speaking at a press conference earlier today (Thurs) the Terriers boss David Wagner said subject to personal terms the deal for Pritchard to move to West Yorkshire has been agreed with Norwich.
Peterborough United have signed winger Joe Ward from National League side Woking.
The 22-year-old joins the League One side on a two-and-a-half year deal.
He featured in Woking's FA Cup draw with Posh last month, scoring one of his seven goals in 31 games in that tie.
He told the club website: "I was keen to sign. I have got good people around me, they have been great in getting the deal done and I cannot wait to get started. Peterborough are a side that like to attack and that suits me."
The former Chelmsford, Brighton and Lincoln player is available for Saturday's game at Wigan.
