The death of a railway worker in Essex has led to Network Rail being fined £125,000 for breaching health and safety laws.
64-year-old Malcolm Slater died in 2008 when the aerial platform he was working on sheared off. Two other workers suffered lasting injuries as Serena Sandhu reports.
