A Serious Case Review has concluded that an "error of judgement failed to protect twins from harm".
A mother in Lincolnshire abused her twin babies, despite having previously had four children removed from her care.
The mother, whose identity has not been revealed, was jailed in 2005 for wilful neglect and Grievous Bodily Harm to her eldest son.
In May 2011 one of the twins, a baby girl, was admitted to hospital with life-threatening head injuries. It was then discovered that her brother had a healing fracture to the left clavicle. The pair were just 7-weeks-old at the time.
They are both are now making good progress since being removed from their parents care. They have both since been adopted.
The report’s author concluded that: “There was a failure by the social worker responsible to assess and evaluate fully the information available”. The author also concluded there was a failure of other agencies to challenge the plan put forward by the social worker.
