Andrew Jones (Pies) in Huddersfield has been ordered to pay almost 375,000 pounds for breaching health and safety regulations after a supervisor died in a huge explosion. It happened on Good Friday 2009 and led to the concrete roof of the oven room collapsing on those inside.

37-year-old David Cole from Halifax was killed in the blast. Another worker was also injured.

"The company failed dismally and these failures substantially led to the death of Mr Cole," said Judge Guy Kearl, QC. He added, "Mr Cole was not to blame and this was an ongoing situation rather than an isolated incident."
