When Zahir Aziz was released from jail, he was wrongly assessed as low risk - and it cost the life of his uncle, Shabir Choudary.
Mr Choudary's family say he would still be alive today, had the probation trust in West Yorkshire done its job. Zahir Aziz was under the supervision of probation staff when he went on to kill.
