The findings of the Serious Case Review of the Daniel Pelka case have been published.
The 4-year-old was abused and starved to death by his mother Magdalena Luczak and her partner Mariusz Krezolek.
The purpose of the review was to find out why teachers, doctors and social services didn't prevent his death. It also reveals that police were called to Daniel's home 27 times because of domestic violence in the time before his death.
