Emergency crews were called to the southbound stretch of the M23 at junction 9 near Gatwick at about 4am today following the crash involving a silver Mercedes C220 saloon car, which is believed to have been stationary, a green Honda Accord saloon and a silver Renault Megane, Sussex Police said.
West Sussex Fire and Rescue Service had to cut one man free from one of the vehicles and all four casualties were taken to the Royal Sussex County Hospital.
The driver of the Mercedes was arrested at the scene on suspicion of dangerous driving and being unfit to drive through drink or drugs.

The Highways Agency has advised drivers to follow the diversion route which is in place and said that passengers for Gatwick Airport should remain with their vehicles and follow the diversion route.
Four people were taken to hospital after several vehicles collided on a stretch of motorway early today.
Emergency crews were called to the southbound stretch of the M23 at junction 9 near Gatwick at around 4am following the crash, Sussex Police said. Three vehicles were involved in the collision.
The carriageway was immediately closed and is expected to remain shut until later this morning while forensic examinations take place, a police spokesman said.
The conditions of those caught up in the accident is not known.
Bosses at Gatwick Airport today said it could be handling around 45 million passengers a year by 2030. The airport currently has agreed that it will not build a second runway by 2019 but could build one after that. The airport currently handles 34m passengers each year.
Gatwick chiefs said a noise insulation scheme for local residents already existed and a new scheme would be rolled out in 2013 following a public consultation.
