A major review has been launched into radiology services at hospitals across the country after it was revealed two people died and another was left seriously harmed. X-rays were't properly checked at Portsmouth's Queen Alexandra Hospital.

Portsmouth Hospitals NHS Trust has been told it must take immediate action to ensure patients' X-rays are reviewed by trained clinicians.

During an inspection in May, the Care Quality Commission found 'poor' governance procedures. The Trust has apologised to the families of three patients, two of whom died, for the delays. An independent investigation has been ordered.
