The US military says more than 100 civilians were killed in a blast in Mosul in March.
A Pentagon investigation has found that the civilians were killed when a US airstrike inadvertently triggered a second explosion, causing a building to collapse.
The blast took place on March 17 in the city in the north of Iraq, the report said.
