These proposals have been agreed by health officials and will go before Northumberland County Council members next month.
County Council leader Peter Jackson said:
Northumbria Healthcare NHS Foundation Trust chief executive Jim Mackey added:
“It has been a long, at times frustrating, road but I can honestly say that we have arrived at the right place.
The plans are due to go before the county council's cabinet on September 5.
