Newcastle Hopsitals NHS Trust has had the most patients who have been harmed by incidents that should never have happened.
Figures from NHS England show 148 never events happened in six months, between April and September. Newcastle Upon Tyne Hospitals NHS Foundation Trust had four of them.
The incidents are regarded by the Government as so serious they should never happen.
