There has been a long-standing technical fault at the level crossing in Oxfordshire where a man was killed today, a source in the railyway industry has told Channel 4 News's Michael Crick.
The source said there has been a fault at the crossing in Yarnton for four years, which Network Rail's maintenance team has been unable to fix.
Mr Crick added, though, that Network Rail has said the crossing was in "full working order".
