A park operator has admitting failings after an 11-year-old boy died falling from a zipwire. 11-year-old Bailey Lonsdale died in April 2011. He'd fallen from a ride at Greenwood Forest Park in Gwynedd and suffered head injuries. An inquest in 2013 ruled his death was accidental.

The inquest into the death of a boy from Blackpool who fell from a zip wire at a Gwynedd adventure park has returned a verdict of accidental death.

Bailey Sumner, 11, died after falling from the SwampFlyer ride at Greenwood Forest Park, near Caernarfon, on Easter Sunday in 2011.

An 11-year-old boy who died after falling from a zip wire at a Gwynedd adventure park had been wrongly attached to the ride, an inquest has heard.

Bailey Sumner from Blackpool died when he fell from the 475-ft long SwampFlyer ride at the Greenwood Forest Park in April 2011.

The ride had opened just a week before, after passing a safety inspection.

The inquest at Dolgellau heard a carabiner clip was connected to a "false loop" in the safety attachment.

The hearing, which is due to last three days, continues.
