The man died just days later after making the three hour journey from Bronglais Hospital in Aberystwyth to Singleton Hospital in Swansea with no medical attention.

The nurse is accused of not carrying out an 'adequate assessment' of the patient and of making a 'retrospective entry' in his medical records a year after he died.
