Men in boat accident "were not wearing life jackets"

One man died and another was taken to hospital after a boat capsized in a North Devon estuary. Police were called to an overturned pleasure boat at Bideford Bar at 11.33am after a nearby boat raised the alarm. Paramedics, the RAF search and rescue helicopter and the Coastguard also attended.

A man, believed to be in his 30s, was pronounced dead at the scene and officers are working to contact his next-of-kin.

Devon & Cornwall Police say the other man had only "minor injuries" and was taken to hospital in Exeter.

Two dogs were also recovered, but one died.

Swansea Coastguard watch officer Marc Lancey said the men were not wearing lifejackets.

"The area is a known hazard locally and widely (but) the sea was quite calm with light winds."

CQC issues warning notice to Devon health trust

A health trust in Devon has been issued with a warning notice by the Care Quality Commission for failing to improve care standards for vulnerable patients.

Devon Partnership NHS Trust had already been told to address concerns relating to people with mental health problems at homes in Dawlish and Exeter.

The CQC also found that a lack of facilities meant some patients had to be treated outside the county.
