ITV's From the Heart campaign started this week, helping to encourage people to understand the importance of being on the Organ Donor Register.
Sophie Clarke, 19, was born with Cystic Fibrosis which steadily got worse until she had a double lung transplant last year.
She joins us on the sofa, along with the family of a girl called Leah Davies who died in a motorbike accident last summer and donated her organs.
Leah's dad Andy, her mum Mandy, and her identical twin Sarah, 19, talk about their experiences.
