Police in the US have launched an investigation after an officer shot and seriously injured a 12-year-old boy who was carrying a toy gun.

Police in the city of Cleveland, Ohio said two officers were called to a playground where the boy was report to have been waving a gun around.

A statement on the Cleveland Police website said he had failed to respond to officers' instructions.

They also said an orange label used to indicate the gun is a replica had been removed.

The department said the two officers had both been placed on administrative leave while the shooting is investigated, which is normal protocol in such incidents.
