Coroner condemns care of woman who died while being treated for mental health

A coroner's returned a damning verdict into the care given to a woman who died whilst being treated for her mental health.

Coroner Martin Oldham said there were multiple failures in Colette McCulloch's care, which amounted to a 'catalyst for disaster.' 35 year old Colette had autism and was living at a care home in Bedfordshire when she walked onto the A1 and was killed by a lorry.

Her mother and father said they felt their daughter's death was preventable.
