The family of a Jerseyman who killed himself three years ago are calling for a serious case review into his death.
Steve Le Masurier was admitted to Jersey's psychiatric hospital with severe anxiety in January 2016.
His wife and daughters have expressed concerns about how quickly he was being discharged given his suicidal state.
When staff sent Steve home on overnight leave, he ended his life.
An independent report was conducted just months after Steve took his own life. It found Orchard House was not responsible for his death but it said there could have been better communication with the family and all patients who leave should have a safety plan in place.
