The children's social services in Southampton have apologised following the death of two brothers.
It comes after three serious case reviews have revealed how ten children in their care were not looked after as social workers could not cope with the high workload.

The two boys, Jayden and Bradley Adams, died after they 'slipped through the net' of the social services and one girl was also found with 92 injuries as well as drugs in her system.
